HOW TO WRITE A PRESCRIPTION FOR A COMPOUNDED MEDICATION?

Rx – Compounded Medication

  • Generic Name of Ingredient (s)/ Strength or Dose (ie. mg or %)
  • Dosage form (ie. Capsule, Troche, Transdermal Gel, Suppository, ) Flavour if applicable
  • Quantity
  • Directions for Use

Examples:

Compounded Medication – Suspension
“Generic Name of Drug” 10mg/ml Suspension (Butterscotch flavour)
Quantity: 30mL
Directions: Give 0.2ml (2mg) daily

Compounded Medication- Capsule
“Generic Name of Drug” 5mg Capsule
Quantity: 30
Directions: Take 1 nocte

Compounded Medication – Troche
“Generic Name of Drug” 0.2mg, “Generic Name of Drug” 200mg, “Generic Name of Drug” 1mg Troche
Quantity: 30
Directions: Take ¼ of a troche bd

Compounded – Cream
“Generic Name of Drug” 5%, “Generic Name of Drug” 10%, and “Generic Name of Drug”5% in Lipoderm
Quantity: 30G
Directions: Apply 0.5G by calibrated measure daily
5G/100ml = 50mg/G

Compounded – Cream

Directions: Apply 0.5G using calibrated measure daily.

Directions for Use: Insert ONE pessary pv nocte for 14 days
